It is the employers’ obligation to do this, so make sure they provide it to you and ask for a copy for your records.

3. Seek medical attention. No matter how minor your injuries may seem, it’s important that you go to the emergency room, or see your primary care doctor, for a checkup. There are injuries that can worsen if they are not treated in a timely manner, and result in temporary or even permanent damage.

4. File your claim for workers’ compensation. Submit your claim as soon as possible. Workers’ compensation will help cover your injuries or illness and provide you with some benefits.
